The Right Care Plan, As Unique As Your Loved One

Cognitive Support PuzzleRightCare dementia and cognitive support is delivered by Right at Home Certified Cognitive Support Program Providers who have received 12 hours of specialized training to deliver an individualized care plan that consists of three components:

gearABILITY: We create a care environment that fosters independence and empowers the client and their current ability to complete activities.

puzzle piecePERSONHOOD: We incorporate the client’s personal history, routines and preferences into the care plan, activities and interactions.

clockLIFESTYLE: We collaborate with the client and family to set goals and create support activities in the areas of nutrition, socialization, awareness and wellness, in order to minimize overall cognitive change.
